Dropped Tool - Wrench falls in Derrick

As noted in the learnings, it is critical that all tool tether components are inspected and routinely checked before use, during use and prior to any transportation of tools and tool bags.

If you are using screw-gate carabiners, consider the potential for inadvertent rotation of the screw gate particularly if it comes into contact with other items during use (e.g. personnel clothing, tool bag, worksite etc).

Sprung-loaded or triple action screw gate carabiners will reduce the likelihood of disengagement, and some devices will also feature highly visual colour banding to indicate the closed / locked position.
